RACI chart

A RACI chart is a table that assigns, for each activity in a project, exactly who is Responsible, Accountable, Consulted, and Informed.

Responsible is whoever does the work; Accountable is the one person answerable for it getting done correctly (there should be exactly one accountable person per activity, even if several people are responsible); Consulted are people whose input is sought before a decision or action; Informed are people who are kept up to date after the fact but don't need to weigh in beforehand. The chart's value is in forcing explicitness — most project confusion traces back to two people both assuming the other is accountable for something, or a stakeholder finding out about a decision after it was made when they expected to be consulted.
